Immunization Requirements
- Dates for two measles vaccines or dates for two MMRs
- Date of one mumps vaccine
- Date of one rubella vaccine
- One completed Meningococcal Meningitis Response Form (see link below)
All students are required to show protection against measles, mumps and
rubella before registration. Persons born prior to January 1, 1957 are exempt
from this requirement.
Requirements are as follows:
- Two dates for two doses of live measles vaccine (the 1st dose must have been
administered on or after the first birthday, the second dose at 28 days later and
after 15 months of age), or a blood test showing immunity.
- One dose of live mumps vaccine administered on or after the first birthday,
or a blood test showing immunity.
- One dose of live rubella vaccine administered on or after the first birthday
or a blood test showing immunity.
Documenation for measles, mumps and rubella must be signed and stamped by
a practitioner/medical doctor.
